There are two categories of individuals considered eligible for service under the MHAP Medicaid eligibility and non-Medicaid
eligibility - based on clinical and financial criteria. Services: Accesses and coordinates the delivery of services and
supports required for individual youth and family in an ongoing relationship. Case management involves outreach
to children and families, connecting them with community agencies and resources
to ensure that all needed services and supports are in place. Two group homes for
male adolescents and one co-ed adult group home available in the area. Clinical

applying for and receiving any assistance programs they may qualify for such as Food Stamps,

noon meal delivered each day by a volunteer. LEGAL AIDE: A lawyer, on a regular basis,

Offers door-to-door transportation for medical, social, personal, recreational, and
educational purposes. Provides transportation for Head Start students. All vehicles are
wheelchair accessible. Available Monday - Friday, 8:15am to 4:30pm. RSVP: Retired Senior Volunteer

teen parents, who are attending classes. Child care lab is hands on with child, parenting classes

just for teen parents. Day care is $2 an hour, $16 a day for infants to two year olds and $14 a
day for two to three year olds.
